How to Prepare for a Telemedicine Appointment; American Academy of Dermatology

Tips from board-certified dermatologists

Consumer demand for virtual health care has exploded during the COVID-19 pandemic. Thanks to telemedicine, patients with new concerns and those with chronic conditions can have an appointment with a board-certified physician from the comfort and safety of their homes. According to dermatologists from the American Academy of Dermatology — a specialty with more than two decades of experience in telemedicine — even after the pandemic subsides, the convenience and efficiency of virtual health care visits means telemedicine is here to stay.

“Telemedicine is a wonderful way to connect with your physician, especially for busy families, those with demanding work schedules, and those who live in remote areas,” says board-certified dermatologist Jennifer David, DO, MBA, FAAD, who founded her own virtual dermatology practice. “However, just as you would prepare for an in-person doctor’s visit, it’s important to spend a few minutes preparing for a virtual visit in order to make your appointment as valuable and useful as it can be.”

To help patients get the most out of their telemedicine appointments, Dr. David recommends the following tips:

1.  Before your appointment, contact your insurance provider to find out if your plan covers telemedicine appointments. Many insurance providers have updated their plans to cover telemedicine visits during the COVID-19 pandemic. However, it’s important to find out what type of telemedicine visits are covered and who can deliver them — such as your dermatologist or someone specified by your insurance provider.

2.  Gather essential medical information. This is especially important if you have a telemedicine appointment with a doctor you haven’t seen before. Knowing your medical history will help your doctor make a diagnosis, decide treatment options and prescribe medicine, if necessary. Ask your doctor’s office if they have any forms you need to fill out before your appointment.

In addition, make a list of the following before your appointment:

  • Medications you take
  • Major illnesses or surgeries you have had
  • Previous health problems
  • When your current symptoms began
  • Your allergies
  • Previous health problems or illnesses that your family members have had, such as cancer, heart disease, or diabetes

You will also want to ask your doctor’s office what type of telemedicine visit will be conducted and how:

  • For video visits, you will be sent a website link to connect at the time of your visit.
  • For telephone visits, you may be given instructions on when to expect a call from your doctor.
  • For visits through photos only, contact your doctor’s office to find out when and how to send your pictures. You may also be asked to send photos for video and telephone visits.

3.  Write down any questions you may have. Doing this before your appointment helps you remember everything you plan to talk to your doctor about and make the most of your appointment.

4.  Take pictures. If your concern is something that can be seen on the skin, hair or nails, take clear pictures of the area(s) you need examined. Just like an in-person doctor’s appointment, if you need to have your face or nails examined, remember to remove all make-up or nail polish before taking any pictures. Regardless of how your virtual appointment is conducted (video, telephone, etc.), it can be very helpful to send photos of your skin to your doctor. Your doctor may provide you with instructions to send photos.

To take pictures in the highest-possible quality, follow these tips:

  • Make sure your pictures are well lit. Whenever possible, take your pictures in natural light. If natural light isn’t possible, use another light source, such as a book light or flashlight. Be mindful of shadows or glare on the area(s) of interest.
  • Take multiple pictures, including one of each side of the area(s) you need examined. Make sure to show the entire body part where the affected area is present.
  • Take pictures to compare. For example, if you have a spot on your hand, take pictures of both hands so your doctor can see how that area usually looks. Make sure you also take a close-up and far-away picture of all areas involved so your doctor can compare.
  • Get help. If you live with someone, ask them to take pictures of hard-to-reach areas, like the back. If you live alone, use a mirror to make sure you are taking pictures of the right spot.
  • If your picture turns out blurry, delete and retake it, as a clear picture is critical for an accurate exam.

5.  Avoid irritating your skin before your appointment. Try to avoid doing things that could change the appearance of your skin before your telemedicine appointment. Some examples of things to avoid before your appointment include taking a hot shower, rubbing or picking at your skin, or applying skin care products.

6.  Find a quiet and private space to have your appointment. Make sure you can connect to the internet in that space and minimize any distractions, such as background noise. If your concern is something visible, make sure the space has the best lighting possible.

“While telemedicine can’t always replace an in-person visit with your dermatologist, it can help in times when you can’t get to their office,” says Dr. David. “After your appointment, depending on your condition, your dermatologist may schedule another appointment with you — either in-person or another telemedicine appointment — to see if your condition is improving. Make sure to keep any follow-up appointments that your dermatologist recommends in order to stay healthy.”

These tips are demonstrated in “How to Prepare for a Telemedicine Visit,” a video posted to the AAD website and YouTube channel. This video is part of the AAD’s “Video of the Month” series, which offers tips people can use to properly care for their skin, hair and nails.

This last year has been a remarkable and challenging year for everyone. We have all experienced incalculable loss — of life, of opportunity, and of our daily patterns of living. For the community NEXT for AUTISM proudly serves, the isolation brought on by COVID-19 restrictions has only magnified the many disparities that individuals with autism face at work, at home, and in our communities.

NEXT for AUTISM is keenly aware of how disconnected from their larger communities individuals with autism and their families often feel, even in the best of circumstances. With renewed demands for social justice, calls for greater equity, and increased focus on diversity across our country, it is also time for autistic individuals to not only be seen, but to join in, to be included in this important movement toward acceptance, representation, and true belonging, no matter our perceived differences.

This April is different than in past years when it comes to autism. NEXT for AUTISM and our colleagues across the disability community are formally calling for a change to the terminology we use to recognize this month, moving away from the previous moniker of “Autism Awareness Month,” to the more relevant and meaningful “Autism Acceptance Month.” This shift speaks volumes, and not in name alone.

Autism Acceptance Month signifies a necessary evolution in expecting a deeper understanding, value, and inclusion of individuals with autism in society. It also ushers in a new era for NEXT for AUTISM as an organization intent on growing our mission impact and core areas of work to best support autistic individuals. Adults living with autism deserve equity in the workforce, homes that are safe and welcoming, a rich social life—life experiences that all adults crave.

NEXT for AUTISM is a critical part of the solution. We are committed to building needed resources that promote opportunities and choice and truly move the needle to make foundational changes for adults with autism. “When our late mother, Peggy, was fighting for her life against this preventable infection, I and my brother were scouring the Internet for information on C. diff,” said PLF’s Executive Director Christian John Lillis. “Unfortunately, what we found was either outdated or in the form of medical and scientific literature that was not easy to understand. While our website has long been a valuable resource for C. diff patients and their caregivers, we want to provide all the key information in one place.”

The Care Guide was funded by Bio-K+, a division of Kerry. The Guide includes easy to understand information on topics C. diff patients frequently ask about, including how C. diff is spread, getting an accurate diagnosis, treatment options for primary and recurrent C. diff, preventing C. diff spread in the home, and socio-emotional impacts of recurrent C. diff.

E-Technology Satisfying Both Patients and Clinicians in a Study of 104 Patients Trying to Conceive

At-home based hormonal testing kits and mobile tracking applications uncover more effective testing and data interpretation for women’s health.

Online or smartphone fertility tracking apps are “satisfying both clinicians and patients” for timelier virtual care. Patients monitoring their menstrual cycles with cycle tracking apps, along with daily tracking of urinary sex hormones or cervical secretions, contribute to quicker diagnoses and treatments. Innovative e-technology can now respond to concerning conditions around “irregular cycles, infertility, hormonal disturbances and natural birth methods” with aid from virtual apps’ data. The findings are published in the open-access journal The Annals of Family Medicine.

Dr. Rene Leiva and Dr. Lucas DiRienzo conducted clinical research on 104 patients, tracking their menstrual cycles according to fertility and menstrual tracking apps, like the Premom Ovulation Tracker app. For the purposes of ovulation prediction, the study included daily tracking of urinary sex hormones, cervical secretions, or a combination of both. 82% of patients were tracking due to infertility/health reasons, while 18% used this method for family planning.

Clinicians received patients’ cycle data electronically from Premom, allowing for medical interpretation and opportunities for earlier prevention of emerging health issues. Ovulation confirmation was confirmed using lab-based serum progesterone. 

Findings are very satisfying for both patients and clinicians to remotely monitor the menstrual cycle and for patients to be prepared for doctors’ visits in person or virtually. The doctors stated in the article, “We can supervise for normalization of irregular cycles in polycystic ovary syndrome (PCOS) and in adolescents, resolve unusual uterine bleeding, and diagnose fertile window or anovulation in subfertility.” Daily at-home hormonal testing such as ovulation predictor kits are making it possible for early intervention to combat common fertility conditions.

The pandemic has expedited the need of virtual technology. Healthcare providers are beginning to value virtual care to more regularly monitor their patients’ monthly cycles. Mobile tracking apps are beginning to advance women’s reproductive health for all.

World Hearing Day – Amplifon Confirms Commitment to Ongoing Prevention & Treatment of Hearing Loss

Amplifon is proud to celebrate World Hearing Day, confirming its commitment to the treatment and prevention of hearing loss, a condition that impacts nearly 40 million Americans.

According to new research from the World Health Organization, by 2050, the number of individuals who will be living with some degree of hearing loss is set to reach 2.5 billion. As such, identification and treatment have never been more important, especially considering almost 60 percent of hearing loss in children is due to causes that can be mitigated through preventative measures.

For this reason, Amplifon is committed to promoting the importance of prevention and proper education on responsible listening and is incredibly proud of the work of our Miracle-Ear Foundation, which provides adults and children free hearing aids and a lifetime of qualifying services at no cost. Since its founding in 1990, the Miracle-Ear Foundation has donated more than 30,000 hearing aids to more than 16,000 children and adults nationwide, who would otherwise not be able to afford them.

The difficulties for those suffering from hearing loss increased in 2020, with the start and ongoing spread of COVID-19. The pandemic brought increased attention to the importance of hearing in remaining connected with friends and families, with the necessary use of face masks making communication more challenging for those suffering from hearing loss.

To ensure our communities had ongoing access to essential hearing care services in our Miracle-Ear stores, Amplifon developed a new operative model in collaboration with internationally renowned experts from the Columbia University College of Physicians and Surgeons to implement rigorous protocols and safety standards in our stores.

American Kidney Fund Applauds Introduction of Federal Living Donor Protection Act of 2021

The American Kidney Fund (AKF) applauds the introduction of the federal Living Donor Protection Act of 2021 (H.R.1255/S.377), which would provide living organ donors important job protections while also protecting them from discrimination by life, disability and long-term care insurance carriers.

The federal legislation would provide a uniform baseline of protections nationwide for living organ donors. AKF’s State of the States: Living Donor Protection Report Card, released in February, found that the 50 states and the District of Columbia currently offer a patchwork of protections to living organ donors-meaning that where a person lives can affect their ability to be a living donor.

There are currently 108,000 people on the national transplant waiting list, including 93,000 who are waiting for a kidney. The bipartisan legislation was introduced in the House by Rep. Jerry Nadler (D-NY) and Rep. Jaime Herrera Beutler (R-Washington) and in the Senate by Sen. Kirsten Gillibrand (D-NY) and Sen. Tom Cotton (R-Arkansas).

“Passage of the federal Living Donor Protection Act would ultimately shorten the kidney transplant waiting list by making it easier for more people to be living donors,” said LaVarne A. Burton, president and CEO of the American Kidney Fund. “AKF has spearheaded efforts at the state level to pass living donor protections, working with state lawmakers to get such laws passed in 12 states. We are grateful to Reps. Nadler and Herrera Beutler, and Sens. Gillibrand and Cotton, for introducing legislation that will give people in every state the same baseline protections-ultimately saving the lives of people who are waiting for a transplant.”

The Living Donor Protection Act of 2021 (H.R.1255/S.377) would ensure that living organ donors have Family Medical Leave Act (FMLA) protections so they can take time off work to donate the kidney and recover from the surgery without fear of losing their job. The U.S. Department of Labor (DOL) issued an opinion letter in 2018 which stated that organ donation is protected under FMLA. The Living Donor Protection Act would codify the DOL opinion, protecting under federal statute those living donors whose employers are covered under FMLA.

The bill would also protect living donors from discrimination by insurance carriers and prohibits insurers from:

  • Declining or limiting coverage of a person under any life insurance policy, disability insurance policy, or long-term care insurance policy due to the person’s living donor status
  • Preventing a person from donating all or part of an organ as a condition of receiving a life insurance policy, disability insurance policy, or long-term care insurance policy;
  • Considering the fact that a person is a living organ donor when determining the premium amount.
